Publicity Authorization Form - For Competitive and All Star teams ONLY! - All teams must complete this form and have it signed by the parents/players if they have a team website or to have an article/photograph printed in a newspaper. Click here for an MS Word version of the form (editable).
Team Activity Authorization Form - Use this form for team activities such as training/conditioning sessions or team luncheons. Not to be used for a function requiring a license.
Team Fundraising Event Request - Use this PSC form to notify the club of your intention to hold an event for team fundraising. Click here for an MS Word version of the form (editable).
Team Non-PSC Training Authorization - All non-PSC training on behalf of a Pickering Soccer Competitive team MUST BE PRE-APPROVED by PSC prior to committment with any outside organization.
Team Roster Form - This form is an excel spreadsheet which you can edit with your player information.
Waiver Player Over 18 - Use this OSA waiver form to register all players aged 18 and over.
Waiver Player Under 18 - Use this OSA waiver form to register all players under the age of 18.
General Forms
Application for OSA Number - Complete the following information and submit it to the Pickering Soccer Club to obtain an OSA number (required for registration for all programs). A copy of proof of birthdate (Birth Certificate, Health Card) is also required for program registrations.
City of Pickering Incident Reporting Form
PSC Special Incident Form - Use this form to report any incidents (including injury) encountered during a Pickering Soccer Club event. This includes incidents which occur during practices or games, on the field or in the vicinity of a PSC event.
Rebate Request - Use this form to request a rebate if you have more than two (2) children registered and playing in the same program.
Refund Request - Use this form to request a refund. Please note: There are conditions that apply - see our Refund Policy under Club Info/Publications/Club Policies.

Recreational Player (Team) Evaluation Form - For use by a Recreational Coach to submit feedback regarding each player on the team.
